I purchased a set of firestone destination le from tire rack in the appropriate time frame, but did not receive a rebate form and now I can not find the form on your site to print out. The rebate was one of the reasons I purchased the tires and wil be upset if I can not redeem this. PLease email me the form or let me now where i can get it. Thanks

Thanks for your inquiry. Our records show we mailed your rebate certificate and duplicate copy of your invoice to your billing address on May 23rd. Since you did not receive it, I would suggest you contact our Customer Service department at 1-800-428-8355 Ext. 360 or via email at custsvc@tirerack.com and they will be able to assist.

Like the other notes above, I purchased a set of four Firestone Destination LE tires on 3-5-2013. I was not given a rebate form at the time or purchase and I am unable to find the form on line. I will try the 800-428-8355 number and customer service .com contact information that you gave to Allan. If there is anything else you can do in the meantime, I would appreciate it. I don't want the deadline to pass.
